does anyone get bespoke clothes on etsy or other platforms?

I have a hard time finding jackets and blouses that fit me because I'm 5'9 with broader shoulders and longer arms and I need to get some blazers and professional ish clothes (closer to smart casual than business). Tall sizes usually fit but I feel like the quality in brands like Eddie bauer, Old Navy, gap etc have gone seriously downhill and the prices are just insane, or things are 100 percent polyester and dry clean only which is not practical because I will be soon moving to a hot climate. Does anyone get clothes made to measure? What are your experiences with etsy and other merchants? I only want to get a few well fitting, high quality pieces and this is getting harder and harder.
